I understand that participation in sports activities, games, and glow-in-the-dark events involves physical activity and carries a risk of injury, including but not limited to sprains, strains, bumps, bruises, or more serious injuries. I voluntarily assume all risks associated with participation in any Glow Up Sports activities or events. 

I agree that I/my child is physically able to participate in the activities and that I will inform Glow Up Sports staff of any relevant medical conditions prior to participation.

In consideration for being allowed to participate in any Glow Up Sports activity or event, I, on behalf of myself, my child, and our respective heirs, assigns, and legal representatives, hereby release, waive, discharge, and hold harmless Glow Up Sports LLC, its owners, employees, agents, contractors, and venue partners from any and all claims, liabilities, or causes of action resulting from injury, illness, or property damage arising from participation in any Glow Up Sports activity.

If any term, clause, or provision of this waiver is found unenforceable under applicable law, then that provision shall be severed from this agreement, and the remaining provisions shall remain enforceable.

I authorize Glow Up Sports staff to administer basic first aid and/or seek medical treatment if necessary. I understand that I am responsible for any medical costs incurred as a result of injury during participation.

I understand that respectful and responsible behavior is expected of all participants. Glow Up Sports reserves the right to remove any participant who behaves inappropriately or poses a safety risk to themselves or others, without refund.

I grant GlowUp Sports permission to photograph or video me/my child during events and use those images for marketing or promotional purposes, including social media, unless I notify Glow Up Sports in writing prior to the event.
